4. Photocopy of certficate of CSC eligibility/PRC rating and PRC license 

5. Training Certificates/TESDA Certificate and

6 Photocopy of Transcript of Records and Diploma, if undegraduate- Certified True Copy of Grades

7. Certificate of Employment (COE)

8. NBI Clearance
